-The Insta-Cast is used to fix a broken limb. You can tell if you've broken a limb because walking around on it will probably hurt you a good deal, and you'll get messages saying so, diagnosing yourself with "diag me" or the "tac" command will confirm it.
-If you're climbing anything higher than 3m or manage to get pushed out an aircraft, you might break a limb in falling down, although you can get broken limbs during combat if your opponent is strong and is using Fists or a Club weapon.
