Rad sickness is acquired from being in radioactive places, like the wasteland or mutation spots. Most of the time, you will just get stomach aches and get annoying messages, but if you venture too deep into the wasteland without protection, you will learn there is a fine line between just having the message and getting 2 radiation mutations that fuck up your stats.
